We've done this a few times. The first time, when they were much younger, we gave our two children small suitcases with various Disney-themed items. We tried to get them to guess what it all meant, but in the end we just had to tell them.
At this point, we've been going annually for about 14 years, so it's kind of hard to surprise them. However, we decided this past year to buy annual passes instead of regular Park Hoppers (thank you, DVC Discount!), and we surprised all three of our kids with a quick trip to Star Wars Weekends back in April. We never told them a thing, or let a single hint drop. We just picked them up from school (we DID let the teachers know ahead of time). I videoed each child's reaction.
You can see those reactions here:

We're going in January, and we're taking my sister and her family. HER kids don't know, and they're going to find out by opening presents....we've given each person in their family a MagicBand. They're going to Skype us, and they'll open it while we watch. Can't wait!
